PHP vs IOP: How to Choose the Right Level of Care

When weekly therapy isn’t enough but a hospital stay feels like too much, two options usually come up: a partial hospitalization program (PHP) and an intensive outpatient program (IOP). At Ventura Wellness Behavioral Health in Thousand Oaks, we offer both, for mental health conditions and for substance use. This guide explains how PHP and IOP differ, who each one tends to fit, and how the choice usually gets made.

If you or someone you love is in immediate danger, call or text 988 (Suicide & Crisis Lifeline) or call 911. We’re an outpatient program, not an emergency service.

PHP vs IOP: The Short Answer

The main difference is how much time you spend in treatment. PHP is the more structured option, with about six hours of programming a day. IOP offers about three hours a day, several days a week. Both are outpatient care: you go home at the end of each day, sleep in your own bed and stay connected to your life outside treatment.

  • PHP: about six hours a day, for symptoms that make ordinary days hard to get through right now
  • IOP: about three hours a day, for people who need real structure but can still keep up with work, school or family
  • Both: individual and group therapy, psychiatric care and a treatment plan built around your goals

What a Partial Hospitalization Program Offers

A partial hospitalization program is the most intensive level of outpatient care. A typical day mixes group therapy, an individual session or skills practice, and time with the psychiatric team when medication needs attention. Because you’re in treatment for most of the day, you get close clinical support without moving into a facility.

PHP is often a good fit if you:

  • are struggling to work, sleep, eat or care for yourself because of depression, anxiety, trauma symptoms or mood swings
  • recently left a hospital or residential program and aren’t ready for a big drop in support
  • tried weekly therapy or IOP and still feel stuck
  • are starting or changing medication and want it watched closely

What an Intensive Outpatient Program Offers

An intensive outpatient program keeps the core of PHP, including therapy, groups and psychiatric support, in fewer hours. That leaves room for a job, classes or family responsibilities. We run morning, afternoon and evening tracks, so many clients from Thousand Oaks, Westlake Village and nearby communities attend IOP without taking time off work.

IOP often fits if you:

  • have symptoms that are persistent and hard to shake, but you’re still managing daily responsibilities
  • are stepping down from PHP and want to keep your progress going
  • want to address drinking or drug use before it gets worse, or need support in early recovery
  • need a schedule that works around your day

How to Choose the Right Level of Care

You don’t have to figure this out on your own before you call. During a short assessment, our clinicians look at several things together and recommend the level of care that matches what you’re dealing with today.

How severe your symptoms are

Panic attacks most days, deep depression, intrusive trauma memories or frequent thoughts of self-harm usually point toward more hours of support. Milder but stubborn symptoms often respond well to IOP.

How well you’re functioning day to day

If you’ve stopped going to work, can’t keep up with basic tasks or are isolating from everyone, PHP gives you more structure. If you’re still showing up but it’s taking everything you have, IOP may be enough.

Substance use and withdrawal risk

Stopping alcohol, benzodiazepines or opioids suddenly can be dangerous after heavy daily use. We don’t provide detox. If you need it, we coordinate a referral to a medical detox program first and plan your start in PHP or IOP for when you finish.

Your support at home and practical needs

A stable home and people who check in on you make outpatient care safer and easier. Work hours, childcare and travel matter too. California residents who can’t come to Thousand Oaks every day can attend through online therapy or a hybrid of in-person and virtual sessions.

Your level of care isn’t permanent. Many clients start in PHP and step down to IOP as symptoms ease; others begin in IOP and step up for a while when life gets harder. Because your plan is reviewed every week, the intensity can change as you do.

What Stays the Same in Both Programs

Whichever program you choose, you’ll have a dedicated therapist, a case manager and a psychiatric team. We build your treatment plan within your first three days and schedule your first psychiatric appointment within your first week. Sessions draw on approaches such as CBT, DBT and EMDR, along with group therapy and medication management. You don’t need a substance use diagnosis to join for mental health care, and if you’re dealing with both, we treat them together.

PHP vs IOP FAQs

Is PHP the same as inpatient treatment?

No. PHP is an outpatient program. You spend most of the day in treatment, then go home each evening. Inpatient and residential programs require you to stay overnight at the facility. PHP gives you a similar amount of therapy during the day while letting you keep your home, your routines and your support system.

Can I keep working while I’m in IOP?

Many people do. IOP runs about three hours a day, and we offer morning, afternoon and evening tracks, so you can often schedule treatment around your job or classes. PHP takes most of the day, so people in PHP usually take leave from work for a while.

How long do PHP and IOP last?

There’s no fixed length. How long you stay depends on your clinical progress, your goals and, in some cases, your insurance authorization. Your treatment team reviews your plan every week and talks with you about when it makes sense to step down to less intensive care.

Does insurance cover PHP and IOP?

Many health plans cover both levels of care, and we’re in network with many major carriers. What you pay depends on your plan’s deductible, copays and authorization rules. Our admissions team offers a free benefits check, and you’ll usually have an answer the same day.

Can I move from PHP to IOP during treatment?

Yes, and many clients do. Stepping down from PHP to IOP is a normal part of treatment once symptoms ease and daily life feels more manageable. You usually keep the same therapist and care team, so the change feels like a shift in schedule rather than starting over.
